ONLY A TALENT AGENT LICENSED PURSUANT TO SECTION 1700.5 OF THE LABOR CODE MAY ENGAGE IN THE OCCUPATION OF PROCURING, OFFERING, PROMISING, OR ATTEMPTING TO PROCURE EMPLOYMENT OR ENGAGEMENTS FOR AN ARTIST. JULIE NATION INC. IS PROHIBITED BY LAW FROM OFFERING OR ATTEMPTING TO OBTAIN AUDITIONS OR EMPLOYMENT FOR YOU. IT MAY ONLY PROVIDE YOU WITH TRAINING. FOR MORE INFORMATION, CONSULT CHAPTER 4.5 (COMMENCING WITH SECTION 1701) OF PART 6 OF DIVISION 2 OF THE LABOR CODE. A DISPUTE ARISING OUT OF THE PERFORMANCE OF THE CONTRACT BY THE TALENT SERVICE THAT IS NOT RESOLVED TO THE SATISFACTION OF THE ARTIST SHOULD BE REFERRED TO A LOCAL CONSUMER AFFAIRS DEPARTMENT OR LOCAL LAW ENFORCEMENT, AS APPROPRIATE.

YOU REPRESENT AND WARRANT THAT NO PERSON HAS TOLD YOU OR LED YOU TO BELIEVE THAT JULIE NATION INC. WOULD PROVIDE ANY SERVICES INCLUDING ANY AUDITION OR POTENTIAL EMPLOYMENT OR CAREER MANAGEMENT OR AGENT SERVICES FOR YOU IN CONNECTION WITH JULIE NATION PROGRAMS/SERVICES OTHER THAN SET FORTH.

Julie Nation Academy is registered and has met the requirements set forth by the California Labor Commissioner and has posted a $50,000 bond pursuant to Section 995.710 of the Civil Code Procedure. McDonald-Leavitt Insurance Agency, Inc., Bond #100349758.

 

RELEASE

I hereby release Julie Nation Inc., its directors, officers, members, agents, employees, independent contractors, interns, staff members, and volunteers from any and all liability not expressly accepted by Julie Nation Inc. I will not hold Julie Nation Inc., its parents, contractors, members, affiliates, directors, agencies, employees, staff members, or volunteers liable for injury, loss, or damage of any kind to me resulting from the rights, consent, and waiver provided by this agreement. This includes during any extracurricular activities including on-location photo shoots, fashion shows, parades, etc. I grant permission to Julie Nation Inc. staff to render or seek emergency first aid treatment for me, if it appears necessary to such person.
